Galleri works by sequencing cell-free DNA circulating in a patient’s blood and using an AI classifier trained on methylation patterns, chemical markers on DNA that behave differently in cancerous versus healthy cells, to flag a signal from more than 50 different cancer types from a single draw. When the algorithm detects a signal, a second model estimates where in the body the cancer likely originated, information doctors say is critical for deciding which follow-up scans or biopsies to order next.
A Complicated Scientific Backdrop
The advisory panel review does not arrive on the heels of unambiguous success. A three-year, NHS-run trial of more than 140,000 asymptomatic adults in the UK reported earlier this year that Galleri failed to produce a statistically significant reduction in late-stage cancer diagnoses compared with standard care, the primary endpoint the trial was designed to prove. Grail has pointed to a favorable trend toward fewer stage III and IV diagnoses across a defined group of twelve cancer types as a partial silver lining, but critics say a trend that misses statistical significance is not the same as proof the test saves lives.
What Grail Is Bringing to the FDA Instead
Rather than resting its case on the UK data alone, Grail’s premarket application leans heavily on its US-based PATHFINDER 2 study, which followed 25,490 consented participants for at least one year, along with early results from the prevalent screening round of the NHS-Galleri trial covering more than 70,000 additional participants. Company executives argue that performance metrics from these larger, more diverse cohorts, including how often the test correctly predicts the tissue of origin when it flags a signal, better reflect how the tool would perform in real-world US screening populations than the UK’s single primary endpoint.
The Commercial Reality Driving Urgency
Galleri is already sold in the US as a laboratory-developed test that consumers and some employers can order without FDA approval, and the numbers suggest demand is real: Grail reported more than 61,000 tests sold in the second quarter of 2026 alone, a 35% jump in volume from the same period a year earlier. But that offering exists in a regulatory gray zone that limits insurance reimbursement and keeps the test out of most standard-of-care screening guidelines. Full FDA approval would open the door to broader physician adoption and, eventually, payer coverage, transforming Galleri from an out-of-pocket option into a mainstream screening tool.
Wall Street Is Watching the Odds Closely
Analysts covering Grail are treating the advisory panel vote as the real test, since the FDA has historically followed its advisory committees’ recommendations in the vast majority of cases. TD Cowen analyst Dan Brennan has put the odds of eventual approval at around 75%, based partly on the historical pattern that the FDA sides with its advisory panels roughly 80% of the time. Grail’s own CEO, Josh Ofman, has told investors he expects a final approval decision in the first part of 2027, assuming the September panel meeting goes well.
