The Permanente Medical Group has completed one of the largest real-world evaluations of ambient AI documentation to date, reporting that more than 7,200 physicians used an AI medical scribe during over 2.5 million patient visits between October 2023 and December 2024, according to an analysis published in NEJM Catalyst. The results, physicians saved nearly 15,800 hours of documentation time, offer some of the most robust evidence yet that ambient AI scribes can meaningfully ease the administrative burden driving physician burnout.

The Documentation Burden Problem

Clinical documentation has long been cited as one of the leading contributors to physician burnout, with doctors routinely spending hours after clinic hours, so-called “pajama time,” completing notes in electronic health records. Ambient AI scribes attempt to solve this by recording patient encounters with consent and automatically generating draft clinical notes for physician review, eliminating much of the manual typing and dictation that has defined medical documentation for decades.

What the Kaiser Permanente Data Shows

Beyond the raw time savings, the study found that 84% of clinicians said the AI scribe improved their communication with patients, and 82% reported greater job satisfaction using the tool. Notably, researchers found no meaningful correlation between a physician’s age, gender or years in practice and their likelihood of adopting the technology, suggesting the tool crossed generational and specialty lines rather than appealing narrowly to younger, more tech-comfortable doctors. Nearly half of surveyed patients also reported noticing improved clinician engagement during visits, indicating the benefits extended beyond physician convenience into the patient experience itself.

How This Fits the National Picture

Kaiser Permanente’s results echo, and in some ways exceed, findings from other major health systems. A Mass General Brigham and Emory Healthcare study of more than 1,400 clinicians found ambient documentation technology reduced burnout prevalence by 21.2 percentage points after 84 days of use, published in JAMA Network Open. Industry-wide, roughly one-third of providers now have access to ambient AI scribe technology, with some institutions reporting adoption well above that average; at UCSF, 70% of physicians were using AI scribes in daily practice as of 2026.

Not Every Study Agrees

Not all evaluations have been as uniformly positive. A separate large-scale study covered by STAT found only modest time savings from AI scribes in practice, with inconsistent usage patterns across different specialties, complicating the narrative that ambient AI is a straightforward win everywhere it’s deployed. Researchers publishing in npj Digital Medicine have also flagged ongoing barriers to scaling these tools across diverse healthcare settings, including current systems’ tendency toward high omission rates and intermittent factual inaccuracies in generated notes that require careful physician review before finalizing.

Balancing Enthusiasm With Oversight

Health system leaders describe ambient AI scribes as an unusually popular intervention in an era when many burnout-reduction initiatives fail to show measurable impact, but they also caution that popularity and time savings do not eliminate the need for rigorous error-checking. Because physicians remain legally and clinically responsible for the accuracy of the final note, health systems have had to build review workflows to catch AI errors before they enter a patient’s permanent medical record.

The Permanente Medical Group says it will continue expanding ambient scribe access and monitoring long-term burnout and quality-of-care outcomes as adoption matures beyond its first full year of data. With adoption already exceeding 2.5 million patient encounters at just one health system, ambient AI documentation appears set to become a default feature of American clinical practice rather than a passing pilot program.

Comparing Adoption Across Health Systems

Kaiser Permanente’s results stand out partly because of how evenly adoption spread across physician demographics, in contrast to some other health systems where usage has skewed toward younger, more tech-comfortable clinicians.
